About Fresh Path Inc
Fresh Path Inc.is a multi service not for profit with the overall goal of supporting Chicago’s most vulnerable and underserved persons and communities with the resources and support needed for residents to live a healthy and stable lives. Fresh Path specializes in working with homeless youth/young adults and youth transitioning out of foster care. Fresh Path is a relatively new agency but management and staff have decades of experience in these fields.
Fresh Path Inc. mission is to provide the residents of our community strong support, community
advocacy and holistic services designed to nurture and strengthen the inner person to promote self-efficacy and empowerment within a family unit.
The purpose of the Fresh Path Inc. programs is to enhance the development of individuals and families with supportive housing programs for homeless youth/young adults, both individuals and families), parenting training, HIV prevention, IDCFS HAP programs for youth transitioning out of foster care peer and parents regaining custody of their children, arts and culture as a healing tool, and other therapeutic care. Our programs are designed to elevate social and psychological functioning of individuals, the standard of living and the quality of life in our community.

Supportive housing
Fresh Path Inc. is committed to providing affordable housing for homeless youth/young adults ages 18-24, both singles and young families. Together with the housing the youth have access to a range of supportive services. This includes intensive case management. Life skills training, employment preparation/placement and therapy. The Fresh Path Inc. programs also use arts and culture as a healing/growth producing tool.
p-Lee station
P-Lee’s Station is a Rapid Rehousing Program for homeless youth ages 18-24. The program serves single youth, parenting youth and young families. When they enter the program, the youth works with the assigned P-Lee Station Case Manager to locate an appropriate apartment in the Chiago community of their choice. The lease for the unit is in the youth’s name. However the program can provide rental subsidies for up to 18 months. The size and length of the subsidies is based on the participant’s income. The P-Lee’s Station Case Manager works with the youth to develop realistic goals and a detailed plan for achieving the goals. This includes linking the youth to internal and external resources. After entering the program, the youth can work with the program Employment Counselor who provides pre-employment readiness services and employment placement when the youth is ready. A program Counselor is also available if the youth indicates any mental health issues that could serve as a barrier to independence and stability.
IDCFS HAP PROGRAM
Fresh Path Inc. has a contract with IDCFS to provide an important service to parents in the process of having children returned to them from foster care placement and to youth transitioning out of foster care. The Fresh Path Inc. Housing Advocates work with the parents and the youth to locate appropriate housing and to secure any vouchers and or subsidies for which they are eligible. They support the parents or youth in securing any needed documents. The Advocate may work with the youth if eligible up to the youth’s 23rd birthday when the youth has transitioned to specific forms of subsidized housing. Once the youth is no longer being served by the foster care program, the HAP Advocate will also support the youth in attaining and retaining employment, attaining benefits, and linking to other needed resources such as health care and mental health services. These services are key in helping the youth overcome barriers to successful independence.
